Brief fertility health history
GP and other medical health providers as appropriate for fertility care.
Any other information you supply to us for the purpose of CrMS Education.

Confidentiality

Personal information provided by you will be treated as confidential. It will be disclosed to a third party only with your prior permission. Medical Consultants may communicate with the Practitioner regarding the management of your chart and/or timing of medical tests as agreed with you and your Medical Consultant. You will be asked to sign an authorisation for release of information, prior, to any information exchange.

The only time we will provide your information to a third party without your knowledge, is when we reasonably believe that this action is required under Australian law.
